Our Program:

Effective February 1, 2021, our Functional Medicine Program is now covered by insurance.

Functional Medicine Initial intake:

New patient paperwork can be picked up at our office, mailed to you, emailed to you per your preference or printed off this website the forms tab.

In the packet you will find:

1)  Medical Release Form:

Fill this form out if you would like us to obtain any medical records for review. Add your name and signature to the form (do not add date of birth or any dates). We will need names and any contact information you may have of the providers from whom you would like us to request records.

2)  Remainder of forms (TVFC New Patient Packet and Functional Medicine Male or Female Intake Questionnaire).

3)  Once completed, these must be faxed back to our office @ 931-363-9563, sent by mail to P.O.Box 907, Pulaski, TN, 38478 or dropped off at our office only.

Do not send the completed forms by email due to HIPPA regulations.

New Patient Functional Medicine Initial Visit Cost:         

Insurance will be billed with no added cost. You will be responsible for any copays or coinsurances or deductibles.

Required before session:

1)  Obtaining pertinent medical records for review before session

2)  Filling out of Functional Medicine Intake Questionnaire for review before session

  Initial Session content:

1)  Extensive dive into history as it pertains to a timeline

2)  Discussions coordinating information obtained from history and questionnaire

3)  Formulation/ Initiation of a Functional Medicine program

4)  Ordering of labs/tests if indicated

 Functional Medicine Follow Up Office Visits:

 Cost: Insurance will be billed with no added cost. You will be responsible for any copays or coinsurances or deductibles.

Follow up Session content:

1)  Review of updated medical Symptom Questionnaire

2)  Review /discussion of any lab results

3)  Update on present condition including any changes based

on initiation of program

4)  Revision of Functional medicine Program

Further Information:

Labs:

The patient is responsible for any costs associated with labs and tests. Labs and tests are filed to insurance when applicable.

Supplements:

Supplements may be recommended as part of your program. The patient is responsible for cost of any medicines and/or supplements.
